Access your Configurations and Preferences.
Find the section Configurations of the nutrition assessment form.
Edit as desired:
You can change the assessment form as often as you like, with each change being saved as a default.
You can choose to hide an entire section or multiple questions from a specific section.
If you remove a question from the form, all forms you send to your next clients will have that question omitted, unless you change the form settings again before you send it again.
Currently, it is not possible to add free-form questions to the assessment form as its purpose is to automate the completion of the tab "Information", which differentiates Nutrium's approach from other available softwares.
When the patient answers the questionnaire, you will automatically see the information in the specific fields - including weight and height. Learn how you can send your assessment form to your clients and check their replies in this article.
